we created a new module with the odoo scaffold and put the reference to the images in the __manifest__:

In the STATIC / DESCRIPTION / folder there is the image icon.png

Personalmente non ho mai messo le reference in manifest. Per default l'icona viene cercata in static/description/icon.png e viene usata nelle application e come icona se è attivato il modulo web-responsive di OCA. Se non funziona forse il formato del png che stai usando potrebbe avere qualcosa che non va, ad esempio trasparenze (alpha) che non so se vengono gestite.
